JR. CAMP TERMS, CONDITIONS, AND LIABILITY RELEASE:

By agreeing to and signing the Jr. Camp registration form, you understand the following:
Class grade assignment will be based on the grade your child completed this past school year. Furthermore, you, being the parent or legal guardian of the mentioned child, permit the staff of Peace Haven Baptist Church Jr. Camp to seek medical attention and make decisions on the care or treatment of the child mentioned on the registration form in your absence until you can be notified and are present.
In signing the Jr. Camp registration form, you , being the parent or guardian of the mentioned child, agree not to hold Peace Haven Baptist Church, its members, Elders, or Deacons liable for any injury, loss, damage, or accident that your child might encounter while participating in Jr. Camp. You agree it is 100% your choice for your child to attend camp.
You realize and acknowledge that your child’s participation in Jr. Camp includes some risks and possible hazards. You understand your child’s participation in Jr. Camp may expose them to risks such as swimming, inflatables, dancing, laser tag, and other various outdoor/indoor games. You also understand and acknowledge that Peace Haven will be required to transport your child via van to certain activities (for example: swimming) as needed, and that all drivers are over the age of 25 per insurance policy.
You hereby assume any such risks that might result in your child’s participation, and you unconditionally agree to hold Peace Haven Baptist Church, its members, Elders, or Deacons blameless for any liability concerning your child’s personal health and wellbeing, or any liability for personal property that might be lost, damaged, or stolen while attending Jr. Camp.
You acknowledge you have carefully read the foregoing and you understand that your signature on the Junior Camp registration form holds Peace Haven Baptist Church, its members, Elders, or Deacons harmless for any liability for injury, damage, loss, accident, delay, or irregularity in the schedule.
